The Institute for Accountable is inviting NAACOS member ACOs to participate in a new initiative to improve care for patients with complex needs. These individuals typically have three or more chronic conditions, functional limitations and may have challenging social or physical environments. Their annual medical spending is three to five higher than other adults. This initiative aims to help ACOs implement proven interventions to reduce spending for their most costly patients while improving their access to care and quality of life.

The Institute has worked closely with a NAACOS member ACO Advisory Group supported by three national health foundations to select two models for a 2019 demonstration project: a home visit initiative and an extensivist program. The Institute is seeking ACO partners to participate in these initiatives. Benefits include technical assistance with program development, learning collaboratives with expert faculty and analytic support for measuring your programís return on investment.

More details can be found on the Program Details page. To be eligible to participate, your ACO must indicate interest by completing a short request for information form for the model(s) of interest. Depending on the level of grant support we expect to invite between 10 and 20 ACOs to participate with final commitments required in the second quarter of 2019.
