The Institute for Accountable is inviting NAACOS member ACOs to participate in a new initiative to improve care for patients with complex needs. These individuals typically have three or more chronic conditions, functional limitations and potentially challenging social or physical environments. Annual healthcare spending for these individuals is three to five times more than other categories of adults making them an important population for ACOs.

The Institute has worked closely with a NAACOS member ACO Advisory Group supported by three national health foundations to select two models for a 2019 demonstration project: a home visit initiative and an extensivist program. The Institute is seeking ACO partners to participate in these initiatives. Benefits of participation include technical assistance, learning collaboratives with expert faculty and support for measuring the impact and return on investment for your ACOs program.

To be eligible to participate, your ACO must indicate interest by completing a short request for information form for the model(s) of interest. Depending on the level of grant support available for the project we expect to invite between 10 and 15 ACOs to participate with final commitments required in the second quarter of 2019.
